Voter Registration Application
Notice: New registrations must be received at least 29 days prior to a scheduled election in order to be valid for that election. In order to register to vote in Florida, a citizen must swear or affirm an oath and sign the voter registration application form. The online form begins the registration process. Mail the form to the: Supervisor of Elections Office, Post Office Box 1119, Titusville, Florida 32781-1119. If your application is complete, and you qualify as a voter, the
Supervisor of Elections will mail you a registration identification card as
official notification of your registration. If you have any questions,
